If you haven’t signed up for JC Penney Rewards yet, there is this offer (valid through 3/31/09):

You will be prompted to register a credit card, which is required to earn future rewards, but it’s not required to get the $5 certificate. Just submit without a credit card. After signing up, log into your account and then go to rewards to print out the reward certificate.
